Welcome to Salem India Wellness Community (SIWC), a community program of HWY Foundation. We bring people and families together for social, cultural, educational, and wellness activities. Please complete this form before joining.
By submitting this form, I understand and agree that:
Participation is voluntary. I may choose which activities I or my children attend and may stop participating at any time.
I am responsible for supervising children attending with me unless an event specifically offers a supervised children’s program.
I will treat others respectfully and follow event and venue rules. Organizers may address conduct that disrupts or endangers others.
Activities may involve movement, food, or other ordinary risks. I will decide which activities are appropriate for me and my children. Wellness information shared at events is educational and does not replace individual medical care.
HWY Foundation may use the information I provide to manage community participation and contact me about my registration, membership, events, or important changes.
Community membership, whether free or paid, does not give me voting or other governance rights in HWY Foundation.
