Dandiya/Garba Event Participation Consent and Acknowledgment of Risk
Organizer: Salem India Wellness Community, a community program of HWY Foundation
Event: October 25, 2026, 3–5 p.m., at Center 50+, Salem, Oregon
Please read the following before participating in the event.
Voluntary participation. I choose to attend this event voluntarily. I may decline any activity or stop participating at any time.
Dancing and Dandiya activities. I understand that dancing and using Dandiya sticks involve risks, including slips, falls, collisions, accidental contact from sticks, muscle strains, and other injuries. I will participate within my abilities, maintain appropriate space, use sticks carefully, and follow organizers’ safety instructions.
Children’s supervision. I am responsible for supervising children who attend with me. This event does not provide drop-off childcare. I will help children participate safely and follow event rules.
Food and allergies. I understand that individual participants prepare potluck food. Organizers cannot guarantee ingredients, allergen-free food, or the absence of cross-contact. I will make food choices appropriate for myself and the children in my care. If I bring food, I will identify its ingredients and known allergens to the best of my knowledge.
Respectful conduct. I agree to treat participants, volunteers, and venue staff respectfully and follow event and venue rules. Organizers may ask anyone whose conduct creates a safety concern or disrupts the event to stop an activity or leave.
Health and emergencies. I will consider whether the activities are appropriate for my health and seek medical advice beforehand if needed. If an emergency occurs, organizers may contact emergency services and my emergency contact.
Registration information. HWY Foundation may use the information I provide to administer this event, contact me about event changes, and respond to safety concerns.
